Leadership and Service

St. Nicholas School offers many after school activities that provide children with opportunities to develop their leadership skills and to share their gifts and talents through service to others. These opportunities enrich our student’s lives and better our community. These activities include:

Form	Description of Use
Health
Emergency Medical	Bill Required for registration - update annually and with changes.
Health History	Required for registration.
Registration
Registration 2024-2025	Required to enroll new student for the new school year.
Re-Registration 2024-2025	Required to re-enroll students for the new school year.
Tuition Contract	Required to re-enroll students for the new school year.
Parishioner Verification Letter	Required for all families that belong to another parish. Included on the registration forms.
Photos / Internet
Internet Policy & Agreement	All parents and students must read annually
Photo/Publication Permission	Parents sign annually to grant/restrict photo and publication use
Communication
Email Authorization	Parents sign to authorize school to contact you by email
Volunteer Sheet	To be completed by parents/guardians.
Student Data Sheet	To be completed by parents for each child. Please fill in all information.
Parent/Student Contract	To be signed by parents and students after reviewing Handbook and Parental support statement
Parental Support Statement	To be reviewed annually by parents
Conduct
Student Handbook	To be reviewed annually by parents and students
Code of Conduct	To be reviewed annually and signed by parents and students
